| 摘 要： | 目的探讨Mirizzi综合征(MS)的诊断与外科处理方法。方法对23例MS患者的术前诊断及手术情况进行分析,总结其诊断及外科处理的特点。结果术前23例均行B超检查,确诊为MS4例;22例行CT检查,确诊2例;磁共振胰胆管成像检查18例,确诊6例;内镜逆行胰胆管造影检查10例,确诊2例。术中右肝管损伤1例,肝总管损伤1例,均行损伤胆管修补+胆总管T管引流术;1例因严重胆管炎术后心、肺和肾等多脏器功能衰竭死亡,余22例均随访6个月～3年,均恢复良好。结论磁共振胰胆管成像是诊断MS的理想方法,术中依据病变具体情况采用相应的外科治疗措施,是减少或避免术后胆管并发症的关键。
