3种全子宫切除术的临床应用比较

目的评价经腹全子宫切除术（TAH）、阴式子宫切除术（TVH）、腹腔镜下全子宫切除术（LTH）3种术式的临床应用效果。方法回顾性分析TAH40例、TVH35例、LTH40例的临床资料。结果 3组患者无并发症发生,3种术式中,在手术时间上3组相比差异无统计学意义（P〉0.05）,在术中出血量、术后排气时间、术后住院时间上,TAH组与其他两组相比,差异均有统计学意义（P〈0.05）。结论 TVH、LTH创伤小、患者痛苦少、术后恢复快。3种术式均为子宫切除的有效术式,各有利弊,应根据患者的具体情况选择合理术式。

Clinical comparative study of three kinds of hysterectomy

Objective To investigate the safety and superiority of three kinds of hysterectomy.Methods 40 women undergoing transabdominal hysterectomy(TAH),35 undergoing transvaginal hysterectomy(TVH),40 women undergoing total laparoscopic hysterectomy(LTH)from Oct,2007 to Oct,2009 were studied retrospectively.The intraoperative time,blood loss,and recovery of three groups were compared.Results No difference exsisted in the intraoperative time of three groups(P>0.05).The intraoperative blood loss,hospital stay,time to full of recovery in LTH group and TVH group were less than those in TAH group obviously(P<0.05).Conclusion TVH and TLH are safe,effective and reproducible procedures and can be performed widely,but different operation should be selected by the character of different cases.
