52例脊柱结核的外科手术治疗体会

目的探讨脊柱结核手术治疗的方法及其临床疗效,总结脊柱结核的治疗经验。方法回顾性分析2006年2月至2010年2月我院所收治的52例脊柱结核病患者的临床资料。结果平均手术时间为3h,平均出血850mL。全部伤口均能一期愈合,术后2周即可起床活动,180d后可见椎板或椎间植骨轮廓不清,病灶处骨缺损已基本消失。伴有神经受损者中,2例Franke1分级从B级恢复到D级,其余50例均恢复到E级。术后所有患者后凸畸形都得到矫正。结论有效应用抗结核药物,是手术成功的保证。彻底病灶清除、植骨、坚强的内固定可使后凸畸形得到矫正、并有利于植骨融合、重建脊柱的稳定性。

52 Cases of Spinal Surgical Treatment of Tuberculosis

Objective To evaluate methods of surgical treatment of spinal tuberculosis and its clinical efficacy,summed up the experience of treatment of spinal tuberculosis.Methods Retrospective analysis of February 2006 to February 2010 I institutions treated 52 cases of spinal tuberculosis in patients with clinical data.Results The mean operation time was 3h,the average blood 850mL.All wounds can a healing after 2 weeks to get up activities,180d seen after laminectomy or interbody profile is unclear,bone lesions have disappeared.Persons associated with nerve damage,2 Franke1 grade class back to D from B grade,the remaining 50 patients were restored to the E-class.Kyphosis after all patients have been corrected.Conclusion The effective application of anti-tuberculosis drugs to ensure a successful operation.Radical debridement,bone grafting,rigid internal fixation can be corrected kyphosis,and facilitate fusion,reconstruction of spinal stability.
