不同手术方式治疗原发性髋臼周围骨盆恶性肿瘤疗效比较

目的比较不同手术方式治疗原发性髋臼周围骨盆恶性肿瘤疗效。方法对15例手术治疗的骨盆肿瘤进行回顾性分析,人工半骨盆置换4例,内半骨盆切除9例,半骨盆切除2例。结果随访6-60个月,按Enneking方法进行评估：人工半骨盆置换组（4例）：优1例,一般2例,差1例;内半骨盆切除组（9例）：良5例,一般2例,差1例,死亡1例;半骨盆切除组（2例）：死亡2例。人工半骨盆置换和内半骨盆切除组疗效差异无显著性（P〉0.05）。结论人工半骨盆置换手术困难,内半骨盆切除仍是治疗骨盆肿瘤的有效方法。

Comparison of the results of different procedures for primary periacetabular malignancy in pelvis

Objective To compare the effects of different procedurs for primary periacetabular malignancy in pelvis.Methods Fifteen patients with primary periacetabular malignancies treated surgically were reviewed, which included prosthetic hemipelvic replacement: 4 cases; internal hemipelvectomy: 9 cases;hemipelvectomy 2 cases.Results All patients were followed up for 6 to 60 months.The results were evaluated according to the Enneking system:1 of 4 patients with prosthetic hemipelvic replacement was excellent, 2 fair,1 poor; 5 of 9 patients with internal hemipelvectomy were good,2 fair,1 poor,1 died at the final followup; all 2 cases with hemipelvectomy died at the final followup. There was no significant difference between prosthetic hemipelvic replacement and internal hemipelvectomy.Conclusions Prosthetic hemipelvic replacement for pelvic tumors is very difficult, and internal hemipelvectomy is still an effective way for pelvic tumors.
