冠状动脉介入治疗对冠心病患者血浆高敏C反应蛋白和基质金属蛋白酶-9水平的影响

摘    要:目的通过测定冠心病患者行冠状动脉介入治疗前后血浆高敏C反应蛋白(hs-CRP)及基质金属蛋白酶-9(MMP-6)的血浆浓度,分析冠状动脉支架术对冠心病患者炎症指标的影响。方法选择经冠状动脉介入治疗单支病变的冠心痛患者70例作为介入组,40例经冠状动脉造影证实冠状动脉正常的人作为对照组。分别采用免疫浊度法和酶联免疫吸附法检测入选者冠状动脉介入治疗前后hs-CRP和MMP-1水平。结果介入组患者术后血浆hs—CRP水平显著高于术前的,而对照组冠状动脉造影术后hs—CRP与术前的相比无显著性差异(P〉0.05)。介入组患者术后血浆MMP—1显著高于术前,而对照组冠脉造影术后MMP-1与术前的相比无显著性差异(P〉0.05)。结论冠脉介入治疗可促进冠心病患者血浆hs-CRP及MMP-1水平的升高,是否为冠脉介入治疗术后支架内再狭窄的重要机制尚待进一步考证。

Abstract:Objective It is to determine the plasma contents of high sensitive C -reactive protein( hs- CRP) and matrix metal loproteinase -9 (MMP -9) before and after percutaneous coronary interventional procedures(PCI) in patients with coronary artery disease(CAD), and to evaluate the impact of PCI on the inflammatory indexes in the patients. Methods 70 patients undergoing PCI procedures for CAD compromising a single coronary artery were selected as intervention group. 40 healthy individuals with normal findings by coronary angiography were selected as the control group. Before and after PCI or coronary angiography, plasma hs - CRP and MMP - 9 were measured in all the subjects by immunonephelometry and enzymelinked immunosorbant assay(ELISA) respectively. Results In the CAD patients, the plasma hs- CRP level was significantly higher after PCI than that before PCI, but in the control group ,there was no significant difference in the hs -CRP levels before and after PCI. PCI procedures also lead to significant elevation of plasma MMP -9 level after PCI in the CAD patients, but in the control group, MMP - 9 showed no significant variation after coronary angiography ( P 〉 0.05 ). Conclusion Plasma hs - CRP and MMP -9 are elevated in CAD patients following PCI procedures, but their roles in the vascular restenosis following the procedures need further investigation.
