隆鼻术后并发假体外露的分析与防治

目的：探讨隆鼻术后假体外露之原因及预防措施，改善或提高隆鼻手术效果。方法：本组共98例隆鼻术后假体外露或有明确外露表现者，均手术取出假体，并对假体露出的部位及原因进行分析，提出手术注意事项及预防措施。结果：98例假体外露或有明确外露的患者中，鼻尖部44例(44．9％)，鼻翼缘内侧切口处19例(19．4％)、鼻根部9例(9．2％)、鼻小柱7例(7．1％)、鼻前庭5例(5．1％)及鼻梁5例(5．1％)，两个及两个以上部位均出现外露者9例(9．2％)。结论：鼻尖部是隆鼻术后假体外露最常见的部位。外露的原因有假体雕塑不合适，腔穴分离层次不当，假体放置偏斜、扭曲，创缘缝合不佳及愈合不良等。

The prevention and treatment of the exposure of prosthetics after augmentation rhinoplasty

Objective To investigate the reasons and resolutions of the exposure of prosthetics to improve the outcome of augmentation rhinoplasty. Methods Analyse the sites and the reasons of prosthetics exposures of 98 patients, and bring forward the cautions and precautions derived from them. Results 98 cases in all, in which there are 44(44.9%)nose tip exposures, 19(19.4%) alar rim exposures, 9 (9.2%) nose root exposures, 5 (5.1%)vestibule exposures, 7(7.1%) columella exposurs and 5 (5.1%) bridge of nose exposures. There are 9 cases (9.2%)presenting at least 2 exposure sites. Conclusion The commonest exposure site is nose tip. The main reasons for exposure are the bad sculpture of the prosthetics, the wrong dissecting plane, the distortion of the prosthetics, bad closing and bad healing process.
