摘    要:目的 研究新生儿过敏性肠炎临床表现,并与新生儿坏死性小肠结肠炎(NEC)鉴别,提高其诊治水平。方法 回顾性分析1例类似NEC表现的新生儿过敏性肠炎患儿临床资料,复习相关文献。结果 患儿为44 d女婴,生后4 d开始完全人工喂养后出现腹胀、呕吐、血便等症状,腹部影像学呈NEC表现,禁食后临床征象缓解,并反复发生。进一步行食源性过敏原测定示牛奶蛋白过敏。经抗感染并换用氨基酸配方奶喂养后所有症状消失。结论 新生儿过敏性肠炎可呈NEC表现,临床上需注意鉴别。

Abstract:Objective To analyze clinical manifestations of neonatal allergic colitis,in order to improve the diagnosis and treatment compared with neonatal necrotizing enterocolitis (NEC). Methods Clinical data of a case with neonatal allergic colitis mimicking NEC was retrospectively analyzed and relevant studies were reviewed. Results A 44-day-old infant developed abdominal distension,vomiting,bloody stool over and over again since totally artificial feeding at 4 days of age with abdominal radiological findings similar with NEC,and clinical symptoms relieved after fasting.Further foodborne allergens test showed cow's milk protein allergy.All symptoms disappeared after anti-infection and amino acid-based formula (AAF) feeding. Conclusions Neonatal allergic colitis may show similar symptoms with NEC,so clinical staff should pay attention to indentify different diagnosis.
