“一期三吻合术”治疗胰头癌梗阻性黄疸

目的一期三吻合术治疗胰头癌梗阻性黄疸。方法自1996年6月至2008年10月对39例不能切除的胰头癌,利用完整的空肠顺蠕动与胃、胆囊同时侧侧吻合加空肠侧侧吻合术治疗,解除胆道梗阻和预防性治疗上消化道梗阻。结果术后3d黄疸开始消退,术后3周胆红素正常。平均住院天数21.1d,随访术后生存6个月11例,12～18个月22例,24个月6例。术后无胃排空障碍、吻合口瘘、肠梗阻、黄疸并发症。结论一期三吻合术安全可靠,简便省时,易于掌握,疗效好,有临床应用价值。

One-stage three anastomosis surgical treatment carcinomo of the head of pancreas with obstructive jaundice

Objective One-stage three anastomosis treatment carcinoma of the head of pancreas with obstructive jaundice.Methods From June 1996 to October 2008 for 39 cases of carcinoma of the head of pancreas can not be excised using intact jejunal anterograde peristalsis and a stamach ,gallbladder line at the same time increase the jejunum side-to-side anastomosis ,to release biliary obstruction and prevent upper digestive tract obstruction. Results It after three days of the operation jaundice began to subside ,three weeks after the bilirubin return to normal. the average length of hospital stay was 21.1 days ,follow-up to survive after 6 months 11 cases ,12～18 months 22 cases ,24 months 6 cases.postoperations there is no complications such as delayed gastric emptying ,anastomotic leakage ,intestinal obstruction ,jaundice. Conclusion One-stage three anastomosis is a safe ,simple and time-saving ,easy-to-grasp ,the effect of good surgical techniques.
