非小细胞肺癌淋巴结转移规律的临床分析

目的 探讨原发性非小细胞肺癌的肿瘤大小、病理类型、细胞分化程度、部位类型与淋巴结转移的关系。方法 对61例非小细胞肺癌行肺切除和淋巴结清扫术患者进行临床病理分析。结果 清除淋巴结922枚，阳性率40．8％。原发性肿瘤大小（T）因素中T1、T2、T3患者淋巴结转移率分别为17．2％、38．4％和55．2％（P〈0．01）。中、低分化鳞癌淋巴结转移率分别为22．7%和42．1％（P〈0．01）。中、低分化腺癌淋巴结转移率分别为39．7％和55．9％（P％0．01）。中央型与周围型肺癌的淋巴结转移率分别为44．8％和36．3％（P〈0．01）。腺癌淋巴结转移率显著高于鳞癌（P〈0．01）。结论 非小细胞肺癌的淋巴结转移与原发肿瘤大小、病理类型、细胞分化程度、肿瘤发生部位均有密切关系。

Clinical study on the metastatic patterns of lymph node in non-small cell lung cancer

Objective To investigate the relationship between the size of primary tumor, pathologic classification, cell differentiation or location of tumor and lymph node metastasis in lung cancer. Methods Sixty-one patients with lung cancer underwent premmonectomy or lobectomy and lymphadenectomy. The law of lymph node metastasis was analyzed. Results A total of 922 lymph nodes were removed, metastatic rates was 40.8%. The total lymph node metastatic rates of T_ 1, T_ 2 and T_ 3 diseases were 17.2%, 38.4% and 55.2%. The lymph node metastatic rates in moderate and poor-differentiated squamous cell carcinoma were 22.7% and 42.1%. respectively (P< 0.01).The lymph node metastatic rates were 39.7% and 55.9% in moderate and poor-differentiated adenocarcinoma respectively (P< 0.01). The total lymph node metastatic rates in central and peripheral lung cancer were 44.8% and 36.3% respectively (P< 0.01). The metastatic rate of adenocareinoma was much higher than that of squamous cell carcinoma (P< 0.01). Conclusion The frequency of lymph node metastasis significantly correlate with size of primary tumor, pathological classification, cell differentiation and location of tumor.
