在体脑胶质瘤的氢质子磁共振波谱分析

摘    要:目的:探讨在体脑胶质瘤的1HMRS特点及其病理分级的相关性。方法:将18例经MRI诊断并经病理证实的脑胶质瘤分成3组:良性组、过渡组和恶性组,采用2.0T,PRESS序列对肿瘤和对侧正常组织进行定位,测NAA、Cr、Cho和Lac波。结果:胶质瘤的波谱表现为NAA下降,Cho上升,12例有乳酸波。NAA?Cr显示良性组与恶性组之间存在显著性差异;Cho?Cr和NAA?Cho显示良性组与过渡组、恶性组之间均有显著性差异(P<0.05)。乳酸的信号与肿瘤的级别无确切相关性。结论:1HMRS可以为胶质瘤的影像分级提供一个定量分析的方法。

Abstract:Objective:To evaluate proton MR spectroscopic pattern of gliomas in vivo and to report characteristic spectral patterns that may be helpful in grading glial tumors.Methods:Single-voxel localized 1 HMRS was performed at2.0T in18patients suspected of having cerebral gliomas on MR images who subsequently underwent craniotomy.A corresponding PRESS(NAA,Cr,Cho and Lac)were obtained from normal-appearing brain parenchyma in each patient for comparison with the spectra from known areas of pathology.And the patients were divided into three groups:benign(grades1and2),transitive(grade3)and malignant (grade4).Results:We observed decreased NAA and increased Cho in gliomas.While Lac signal was detected in12of 18patients.A significant difference existed between the NAA?Cr ratios for gliomas of benign group and for gliomas of malignant group(P<0.05).A significant difference also existed between the Cho?Cr ratios and NAA?Cho ratios for gliomas of benign groups,and transitive,malignant group(P<0.05),but no significant different between transitive and malignant group.And there was no reliable correlation between the lactate signal and the grades of gliomas.Conclusion:Proton MR spectroscopy provides a helpful technique of quantitative analysis for grading of gliomas on MR images.
