妊娠期急腹症腹腔镜治疗

目的评价腹腔镜在妊娠期急腹症的临床诊疗价值。方法总结回顾1997年6月至2004年10月作者所在2家医院收治住院的62例妊娠期急腹症患者应用腹腔镜诊断及治疗的临床资料。结果62例妊娠期急腹症患者中,宫内宫外妊娠16例,卵巢扭转3例,卵巢囊肿蒂扭转5例,卵巢良性畸胎瘤18例,卵巢黄素囊肿4例,急性胆囊炎5例,急性阑尾炎11例。所有患者均在腹腔镜下明确诊断,除1例中转开腹外,余全部手术成功,成功率98.3%。结论腹腔镜在妊娠期急腹症中具有广泛的适应证,手术的最佳时间为孕中期,尤其对需要保留生育功能的患者尤为安全、适用。

Laparoscopy on pregnant women with acute abdomen

Objective To explore the value of laparoscopy on pregnant women with acute abdomen. Methods The clinical data of laparoscopy on 62 pregnant women with acute abdomen from 1997 to 2005 in three hospital were analyzed retrospectively. Results Among the 62 pregnant women, there were 16 of uterine pregnancy with ectopic pregnancy,3 of ovarian torsion, 5 of pedicel torsion in ovarian cyst, 18 of ovarian teratoma, 5 of cholecystitis,and 11 of appendicitis. All the patients were confirmed diagnosis by laparoscopy. Expected that one patient underwent the laparotomy, the rest were treated by laparoscopy with the successful rate of 98.3 percent. Conclusion Laparoscopy operation can be used extensively in pregnant woman with acute abdomen. The suitable operation time is in the metaphase pregnancy. This method is especially fit for those who want to remain bearing ability.
