间歇微脉冲模式超声乳化白内障摘除术后角膜内皮细胞超微形态结构的改变

摘    要:目的 观察间歇微脉冲模式超声乳化白内障摘除术后角膜内皮细胞超微形态结构的改变。方法 采用相互对照方法。选纯种新西兰白兔24只,按间歇微脉冲模式、线性模式超声乳化方法随机分为A、B两组,每组12只,右眼手术。实验组(A组)行间歇微脉冲模式超声乳化,对照组(B组)行线性模式超声乳化。按不同有效超声时间(5秒、10秒及15秒)将A、B两组随机分别分为三组,即A1、A2、A3组及B1、B2、B3组,每组4只。术后即刻和术后1个月分别随机处死2只兔,取术眼角膜,分成两半,分别行角膜内皮细胞扫描电镜、透射电镜观察。结果 间歇微脉冲模式超声乳化术后即刻角膜内皮细胞形态结构轻度改变,表面微绒毛减少,分布不均匀,核染色质不均匀,胞浆内线粒体轻度肿大,内质网扩张,吞饮小泡略增多,术后1个月基本恢复正常。结论 间歇微脉冲模式超声乳化白内障摘除术后角膜内皮细胞超微结构改变小,恢复快。

Abstract:Objective To observe changes in ultrastructure of the corneal endothelial cell after interrupted hyper-pulse mode phacoemulsification.Methods Twenty-four New Zealand rabbits (24 eyes) were randomly divided into 2 groups for interrupted hyper-pulse mode and line mode phaco,12 eyes in each,group A (experiment group) for interrupted hyper-pulse mode phaco,group B (control group) for line mode phaco.The operation in right eyes was performed.In each group,the rabbits were again divided into 3 subgroups for the different valid time of ultrasound phaco,group A 1,A 2,A 3,and group B 1,B 2,B 3,four eyes in each.Two rabbits weres randomly executed at 0 hour and 1 month postoperatively.Each cornea of right eye was selected material and divided into 2 parts.Scanning and transmission electronic microscopy in corneal endothelial cell in each part were observed respectively.Results At 0 hour postoperatively,slight changes in morphology were found in cornea endothelial cell.The number of microvilli was decreased and was not well distributed.Transmission electronic microscopy revealed swelling of organelles lightly,increasing vaculoles in the cytoplasm.Ultrastructure was returned to normal 1 month after operation.Conclusions Interrupted hyper-pulse phaco may reduce the corneal endothelial cell lesion and the appearance of endothelium was restored faster.
