脑干听觉电反应的临床应用

摘    要:
测定正常人20例ABR的正常值。测试350例患者的ABR,其中听阈测定244例,用于客观测定婴幼儿及不合作成人的听阈,鉴別语言障碍的原因,器质性聋抑功能性聋。诊断中枢病变28例,对桥小脑角肿瘤,脑干肿瘤、多发性硬化有定位诊断意义。鉴別耳蜗抑蜗后病变78例。双耳Ⅴ波潜伏期差,尤其是双耳Ⅰ~Ⅴ波间期差是诊断的重要参数,应提高Ⅰ波检出率,同时记录耳蜗电图,有助于诊断率的提高。

Abstract:
The values of auditory brain-stem response ( ABR ) were measured in 20 normal subjects and 350 patients. In 214 infants and nonce-operative adults, thresholds were delected objectively in order to identify the causes of speech disorders and differentiate the organic from the functional deafness. Lesions of the central nervous system were diagnosed in 28 cases. It suggeste the significance of ABR in the localization of cerebello-pontir e angle tumour, brain-stem tumour and multiple sclerosis. In 78 cases, differential diagnosis between cochlear and postcochlear deafness was made. It was shown that the latency of wave 5, especially the interval between wave 1 and wave 5, is an important parameter for differential diagnosis. To improve the diagnostic rate, it is necessary to elevate the frequency of revealed wave I and Ecoch G in ABR.
