摘    要:目的 对基层医疗机构9种数字化远程医疗设备使用情况及影响因素进行调查,为推动贵州远程医疗服务工作更好地开展提供政策建议。方法 随机抽取全省207家基层医疗机构(乡镇卫生院和社区卫生服务中心),对其9种数字化远程医疗设备使用情况进行问卷调查;对10家乡镇卫生院负责人进行访谈调查。结果 使用9种设备的基层医疗机构数、检测总人次数均呈逐年增加趋势。至2019年6月,79.7%~88.9%机构使用DR、静态心电、彩超;37.7%~56.5%机构使用全自动生化分析仪、尿液分析仪、电解质分析仪、血球检验仪;动态心电、全科诊疗仪基本未使用;贵州各市州远程医疗设备使用情况有差异,其中六盘水市、黔西南州日常仅使用DR、静态心电、彩超。访谈结果显示,缺乏专业技术人才、就诊人数少、医患双方对远程医疗认知度不高等问题是影响设备使用的因素。结论 数字化远程医疗设备在基层医疗机构的使用情况逐年向好,但仍应重点加强对远程医疗服务的宣传和相应专业人才的培养。在设备配置上不搞“一刀切”,重点加强六盘水市、黔西南州以及就诊人数多、距离较偏远的基层医疗机构的远程医疗服务建设。

Survey on the use of digital telemedicine equipment in primary medical institutions in Guizhou Province

Abstract:Objective To investigate the use of nine types of digital telemedicine equipment in primary medical institutions in Guizhou and its influencing factors, and to provide policy recommendations for promoting better telemedicine services in Guizhou. Methods A total of 207 primary care institutions(township health centers and community health service centers) in the province were randomly selected to conduct a questionnaire survey on the use of nine types of digital telemedicine devices.Leaders of ten township health centers were interviewed. Results The number of primary medical institutions using the nine types of equipment and the total number of people tested showed an increasing trend year by year. By June 2019, 79.7%-88.9% of institutions used DR, static ECG, and color ultrasound; 37.7-56.5% of institutions used automatic biochemical analyzers, urine analyzers, electrolyte analyzers, and blood cell testers; dynamic ECG and general practice diagnostic instruments were basically not used. There were differences in terms of the use of telemedicine equipment in various cities and states in Guizhou, of which Liupanshui and southwest Guizhou only used DR, static ECG, and color ultrasound on a daily basis.The interview results showed that the lack of professional and technical personnel, the small number of consultations, and the weak awareness of telemedicine among doctors and patients were the factors affecting the use of equipment. Conclusion The use of digital telemedicine equipment in primary care institutions is improving year by year, but we should still focus on strengthening the promotion of telemedicine services and the training of professionals. In terms of equipment allocation, we should not adopt a “one-size-fits-all” approach and should focus on strengthening the construction of telemedicine services in Liupanshui, southwest Guizhou, and other remote primary care institutions with a large number of patients.
