初探脑出血患者早期血肿增大的相关因素及其临床意义

目的探讨脑出血患者早期血肿增大的发生率、发生时间及相关因素,从而有利于治疗方案的选择及降低急性期的病死率.方法选择180例经CT复查的脑出血患者,以年龄、入院时平均收缩压（SBP）和舒张压（DBP）、血肿形态、血肿部位、长期大量饮酒、肝功能异常、长期口服肠溶阿司匹林为因素进行比较.结果 180例中35例血肿增大,发生率为19.44%,血肿增大均发生在发病24h内,其中77.14%发生在发病6h内.入院时SBP、血肿形态不规则、长期大量饮酒、肝功能异常、长期口服肠溶阿司匹林与血肿增大相关,有统计学意义P＜0.05.结论应高度重视脑出血患者入院后的病情变化.一旦病情加重,应及时复查头颅CT,及时调整治疗方案.

The related factors and the clinical significance of early enlargement of hematoma in the patients with cerebral hemorrhage

Objective To discuss the incidence, occurring time and related factors of early enlargement of hematoma in the cerebral hemorrhage patients, to choice treatment and reduces case-fatality rate. Methods 180 cerebral hemorrhage patients, that were reexamined by CT, were compared the factors such as the age, the average systolic pressure (SBP) and the diastolic pressure (DBP) hospitalized, the haematoma shape, the haematoma spot, long-term massively drink wine, the abnormal liver function, and taking oral aspirin dissolved in the intestines for a long time. Results 35 of 180 patients had enlargement of hematoma, the rate is 19.44%, all enlargement of hematoma occurred in 24h, 77.14% occurred in 6h. Enlargement of hematoma was correlated with SBP hospitalized, the haematoma abnormal shape, long-term massively drink wine, the abnormal liver function, and taking oral aspirin dissolved in the intestines for a long time ,all these had statistics significance, P<0.05. Conclusion Pay attention to the changes of hospitalized patients with cerebral hemorrhage. Once the condition aggravates, cranial CT should be reexamined and the treatment plan should be adjusted promptly.
