狼疮性肾炎患儿血清总IgE水平变化及临床意义

目的 探讨血清总IgE变化水平在儿童狼疮性肾炎病情活动中的临床意义.方法 采用ELISA法测定狼疮性肾炎患儿和对照组患儿血清中总IgE含量,并检测其24小时尿蛋白定量、尿蛋白/肌酐、血肌酐、血尿素氮,计算肾小球滤过率.结果 狼疮性肾炎组患儿血清总IgE水平较对照组显著升高,差异有高度统计学意义(t=3.889,P<0.01),高IgE亚组患儿公斤体重24小时尿蛋白定量和尿蛋白/尿肌酐水平均明显高于低IgE亚组,差异有统计学意义(t值分别为2.233、2.125,均P<0.05),但两亚组的血肌酐、血尿素氮和肾小球滤过率均无显著统计学差异(t值分别为0.808、1.194、0.522,均P>0.05).结论 血清IgE对判断狼疮性肾炎病情活动有意义,调节IgE表达可能有助于改善狼疮性肾炎患儿的病情.

Clinical significance of changes in total serum IgE level in children with lupus nephritis

Objective To investigate the clinical significance of total serum IgE level change in children with lupus nephritis ( LN). Methods The total IgE levels in serum of children with LN and that in control group were measured by enzyme -linked immunosorbent assay (ELISA).The 24-hour urinary protein, urine protein/creatinine, serum creatinine and blood urea nitrogen were detected , and glomerular filtration rate was then estimated .Results The level of total serum IgE in LN group was significantly higher than that in the control group (t=3.889, P<0.01).The urine protein/creatinine and 24-hour urinary protein levels were significantly lower in children with low IgE level than those with high IgE level (t value was 2.125 and 2.233, respectively, both P<0.05).But there was no significant difference in serum creatinine, blood urea nitrogen and glomerular filtration rate between two subgroups (t value was 0.808, 1.194 and 0.522, respectively, all P>0.05).Conclusion Serum IgE level may be meaningful to predicting disease activity of LN .Furthermore, regulation of IgE expression may relieve the condition of LN in the future .
