MED与ODD治疗腰椎间盘突出症临床分析

目的 比较显微内镜腰椎间盘髓核摘除术(MED)和开窗减压髓核摘除术(ODD)治疗腰椎间盘突出症的临床效果. 方法 148例腰椎间盘突出症患者中70例(MED组)行MED手术,采用旁正中小切口,在内镜下完成神经根探查、减压和椎间盘髓核摘除,78例(ODD组)行ODD手术,采用经椎板间隙开窗、神经根减压和椎间盘髓核摘除.主要观察指标:手术时间、术中出血量、术后卧床时间、髓核重量及住院时间、术后并发症、治疗前后JOA评分及术后疗效等.结果 MED组出血量、术后卧床时间及住院时间均显著少于ODD组,髓核重量及术后并发症组间无明显区别.两组术后JOA评分和改善率均明显提高,MED组术后疗效优良率高于ODD组.结论 MED及ODD术均为治疗腰椎间盘突出症的有效方法,尤其是MED手术具有组织损伤轻和恢复较快的特点.

Clinical comparison between micro-endoscopic discectomy(MED)and open decompression discectomy for treatment of lumbar discherniation
