摘    要:目的 比较3种脂肪乳剂对脂多糖(LPS)诱导急性肺损伤(ALI)大鼠的炎性介质白三烯B4(LTB4)、前列腺素E2(PGE2)和肺泡上皮细胞凋亡的影响.方法 将80只幼年雄性SD大鼠随机分为豆油组(Intra组)、橄榄油组(Clino组)、鱼油组(Omega组)和空白组(Blank组),各组分别采用不同种类脂肪乳剂和0.9%氯化钠溶液处理7 d;并均在取标本前8h时于气管内滴入LPS,建立ALI大鼠模型;同时分别检测各组血清中LTB4和PGE2蛋白含量.观察各组大鼠肺组织的病理改变和检测肺泡上皮细胞凋亡,并计算凋亡指数(Al).结果 (1)Intra组的血清LTB4水平较Blank组显著降低(P〈0.01).而Clino组、Omega组水平均较Intra组显著增高(P〈0.05或0.01);Clino和Omega组血清PGE2水平均较Blank组降低(均P〈0.05),Omega组水平较Intra组降低(P〈0.05).(2)各组大鼠肺组织均可见明显炎症细胞浸润和出血.(3)Clino、Omega组的Al值均较Blank组和Intra组降低(均P〈0.05或0.01),而Clino和Omega组之间AI值的差异则无统计学意义(P〉0.05).结论 Clinoleic和Omegaven具有减轻ALI大鼠炎症反应的作用,并具有调节其免疫功能和减少肺泡上皮细胞凋亡的作用.

Effect of three kinds of lipid emulsions on inflammatory mediators and apoptosis in rats with acute lung injury

Abstract:Objective To compare the effects of three kinds of lipid emulsions on inflammatory mediators and cell apoptosis in rat acute lung injury (ALl). Methods Eighty young Sprague-Dawley rats were randomly divided into 4 groups (n=20 in each): animals were treated with lipoven (intra group), clinoleie (clino group), omegaven (omega group) and normal saline (control group) for 7 days respectively. Then LPS was administrated through trachea to establish ALl, 8h later blood and tissue samples were taken. The serum PGE2 and LTB4 levels were measured by ELISA, lung tissue sections were examined pathologically and apoptosis index (AI)of lung epithelial cells was detected. Results The LTB4 levels of intra group were significantly lower than those of intra, clino and omega groups (P〈0.05 or 0.01). Compared with control group, the PGE2 levels of clino and omega groups were significantly lower (both P〈0.05), and PGE2 levels of omega group were lower than those of intra group (P〈0.05). There were significant inflammatory cells infiltration and bleeding in lung tissue of all groups. Compared with control group and intra group, the AI of clino group and omega group was significantly reduced (P〈0.05 or 0.01); and there was no significant difference between clino group and the omega group. Conclusion Clinoleic and omegaven can relieve inflammation, regulate the immune function, and reduce the apoptosis of alveolar epithelial cells in rats with acute lung injury.
