胃癌脾脏转移5例临床分析

目的：提高对胃癌脾脏转移的认识。方法：回顾分析10余年来复旦大学附属中山医院收治5例胃癌脾脏转移病例的临床资料及随访资料。结果：5例胃癌脾脏转移瘤的原发灶均位于近端胃;其中3例同时有脾脏及其他部位转移,均在短期内死亡;2例发生脾脏孤立转移,经手术切除后获得较长的生存期,其中1例术后3年发生的异期脾脏孤立转移的病例,脾切除后已无瘤生存30余个月。结论：术后密切随诊是早期发现脾转移瘤的重要措施,对胃癌的脾脏孤立转移,脾脏切除是有效的治疗手段。

Splenic Metastasis of Gastric Carcinoma: A Report of Five Cases

Objective：To evaluate the clinical impact of splenic metastasis from gastric cancer.Methods：From the data in Zhongshan Hospital over the last decade,the clinic pathology record of 5 patients with splenic metastasis from gastric cancer were reviewed.Results：All primary tumor were located at the proximal part of stomach;diffused distant metastases were found simultaneously with splenic metastasis in three cases;2 cases with solitary splenic metastasis were resected,one of them was a metachronous solitary splenic metastasis with three years after gastric radical surgery,who had 30 months of disease-free survival after splenectomy.Conclusions：Follow-up is a vital way to identify the splenic metastasis at early-stage.Splenectomy is the most reliable treatment for patients with metachronous and solitary splenic metastasis.
