摘    要:分析颈动脉间隙病变的影像学表现,以探讨该区域病变的鉴别诊断及影像学检查方法的选择。方法19例中,副神经节瘤5例,神经鞘瘤2例,淋巴结转移癌5例,淋巴瘤2例,感染性病变2例,Kimura病1例,颈内静脉血栓2例。CT检查15例,MRI检查4例。

Abstract:Objective Imaging findings of spaceoccupying lesions in carotid space (CS) were analyzed in order to investigate the differential diagnosis of masses in this area and select the proper imaging method in these cases. Methods 19 cases of CS lesions confirmed by histopathology, there were chemodectoma 5, schwannomas 2, metastatic lymphadenopathy 5, lymphomas 2, infectious lesions 2, Kimura's disease 1 and thrombosis of the internal jugular vein 2. 15 cases were examined by CT and 4 cases by MRI. Results Chemodectoma and schwannomas showed softtissues masses located posterior or porterolateral to the great vessels in CS on CT and MRI, the former enhance intensely on CT and presented typical salt and pepper appearance on MRI. On the basis of these findings the latter could be differentiated from chemodectomas. Lymphadenopathy always occrred superficial to the great vessels in CS. Lymphomas had multiple and bilateral lymphadenopathy in the neck. The infectious lesions were widely invasive with unclear boundaries. Conclusions Both CT and MRI are effective methods of localizing neck masses. According to the anatomic location, the pattern of enhancement and relationship with the great vessel in CS, most of the lesions could be identified by CT and MRI. But coronal and sagittal MRI is superior to CT in identifying the lesion in the suprahyoid portion of neck.
