颅底重建59例及文献复习

目的 总结颅底重建的手术经验,探讨重建的目的、方法、材料、手术适应症。方法 回顾分析59例颅底重建病例。颅脑外伤56例,脑膜瘤2例,脑脓肿1例;颅前窝重建54例,颅中窝5例。术中颅底骨折复位,封闭开放鼻窦,修补硬膜,修复颅底缺损,并探查颅神经和减压。结果 全组病人颅腔、眶畸形得以矫正,无并发脑脊液漏、脑膜炎者,颅神经功能改善,面容恢复满意。结论 颅底重建可以改善颅底损伤、颅底病变手术患者的预后,并提高生存质量。

59 Cases of Skull Base Reconstructive Surgery & Bibliographic Review

objective Sum up experience in skull base reconstructive surgery, probe into purpose, method, material and operative indication of the reconstructive surgery Method Review and analyze the 59 patients of skull base reconstruction,including 54 cases of anterior cranial fossa reconstruction and 5 cases of middle cranial fossa reconstruction, for 56 patients of craniocerebral trauma, 2 patients of meningoma and 1 patients of brain abscess Basicranial fracture was repositioned, open accessory nasal sinus blocked, dura repaired, basicranial defects repaired, cranial nerve explored and decompressing made in the course of surgery Result The deformities of cranial cavities and orbital cavities of the whole group were corrected and nobody was complicated by cerebrospinal fluid fistulae or meningitis Cranial nerve function was improved and the physiognomy recovered satisfactorily Conclusion Skull base reconstruction can improve the prognosis of the patients undergoing basicranial injury and pathologic change surgery, and upgrade the survival quality
