高渗氯化钠对红细胞指数的影响

摘    要:本文研究7.5%NaCl溶液在常用量(4ml/kg)时对血液稀释和红细胞本身的影响。选择手术病人15例,平均年龄36.9±10.2岁。手术前采静脉血作为对照,然后经静脉输7.5%的NaCl溶液200~250ml在10~20min内输完(按4ml/kg计算)。立即采取第2次血标本,以后在30min、60min和120min分别采取第3、4、5次血标本,注入含肝素的抗凝试管中,用MicrocelCounter(H-800型)测试计算红细胞参数。结果提示,静注7.5%NaCl后有轻度血液稀释、红细胞平均体积有缩小现象,血红蛋白浓度无变化,不影响红细胞携氧功能。结论:任意加大HS用量,特别在短时间内输入,对红细胞将会产生损害。

Abstract:The influences of 7 5% hyperosmotic saline (HS) 4ml/kg infused iv. within 10 20min on hemodilution and RBCs were studied in 15 patients. Venous blood was sampled at intervals of prior to, immediate after, 30min, 60min and 120min after the administration of HS. RBC indexes were measured with Microcell Counter (Model 800). Results show that HS caused slight reduction of RBC, HGB concentrations, HCT, and MCV, while MCH and MCHC remained unchanged. The above findings denote that O 2 transport function of RBC is not affected by HS at this dose range and infusion rate. Authors suppose that RBC might be damaged by HS at larger dose infused in a short duration.
