液基薄层细胞检测和人乳头状瘤病毒检测在高级别宫颈上皮内瘤变筛查中的检验效能

目的探讨液基薄层细胞检测(TCT)和人乳头状瘤病毒(HPV)检测在高级别宫颈上皮内瘤变(CIN)筛查中的检验效能。方法回顾性分析674例高级别CIN病例的临床资料。所有病例均通过TCT和(或)HPV-阴道镜-活组织检查得到诊断。通过组织学活组织检查对TCT、HPV的检验效能进行回顾性分析。结果 674例CIN患者中,578例行TCT检测,阳性患者509例,阳性检出率为88.06%。508例行HPV检测,阳性患者459例,阳性检出率为90.35%。674例患者中,444例患者同时接受了TCT和HPV检测,二者均为阳性患者350例(78.82%),单一阳性患者90例(20.27%),阳性患者累计为440例(99.09%)。TCT和HPV联合检测阳性率高于单一TCT检测和单一HPV检测阳性率,差异有统计学意义(P<0.01)。结论联合TCT和HPV检测用于高级别CIN的筛查,可以显著提高其检验效能,降低高级别CIN的漏诊率。

Predictive value of thinprep cytologic test and human papillomavirus test in screening of high-grade cervical intraepithelial neoplasia

Objective To explore the predictive value of thinprep cytologic test （TCT）and human papillomavims （HPV） test in screening of high-grade cervical intraepithelial neoplasia（CIN）. Methods The clinical data of six hundred and seventy-four patients with high-grade CIN were analyzed retrospectively. All patients were diagnosed by TCT and/or HPV testcolposcopy-biopsy. The predictive value of TCT and HPV was anlyzed by biopsy. Results Five hundred and seventy-eight patients were examined by TCT and five hundred and nine patients were positive（ positive rate was 88.06% ）. Five hundred and eight patients were examined by HPV test and four hundred and fifty-nine were positive（ positive rate was 90.35% ）. Four hundred and forty-four cases were tested by both TCT and HPV, and the double positive cases were three hundred and fifty（ positive rate 78.82% ）, ninety patients were the single positive（ positive rate 20.27% ）. The aecumulative positive cases were four hundred and forty（ 99.09% ）. The positive rate of combining TCT and HPV test was higher than those of single TCT or HPV test（ P 〈 0.01 ）. Conclusion The combination of TCT and HPV test for the screening of high-grade CIN can improve the diagnostic efficacy and reduce the omission diagnose rate.
