红细胞沉降率与C反应蛋白升高对类风湿性关节炎合并2型糖尿病患者血糖血脂代谢的影响

摘    要:目的探讨红细胞沉降率(ESR)、C反应蛋白(CRP)升高对类风湿性关节炎(RA)合并2型糖尿病(T2DM)患者血糖血脂水平的影响。方法选择62例RA合并T2DM患者作为研究对象,检测血清三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FBG)、餐后2 h血糖(PBG)、CRP、糖化血清白蛋白(GA)、糖化血红蛋白(HbA1c)和ESR。按照ESR是否升高将62例患者分为2组,比较各指标的差异;按CRP是否升高将62例患者分为2组,比较各指标的差异。结果 ESR升高组患者TC/HDL-C高于ESR正常组,差异有统计学意义(P<0.05),GA及CRP水平显著高于ESR正常组,差异有统计学意义(P<0.01);CRP升高组患者HbA1c、GA水平高于CRP正常组,差异有统计学意义(P<0.05),ESR水平显著高于CRP正常组,差异有统计学意义(P<0.01)。结论 RA合并T2DM患者高水平炎症状态可引起血糖血脂代谢紊乱,可能导致其患心血管疾病风险增加。

Abstract:Objective To explore the effect of elevated erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) on blood glucose and blood lipid metabolism of patients with rheumatoid arthritis( RA )combined with type 2 diabetes mellitus(T2DM). Methods Sixty-two patients of RA combined with T2DM as research subjects. The serum triglyceride ( TG), total cholesterol ( TC ), high density lipoprotein cholesterol ( HDL-C ), low-density lipoprotein cholesterol ( LDL-C ), fasting blood glucose ( FBG ), 2 b postprandial blood glucose ( PBG ), CRP, glyeated albumin ( GA ), glycosylatcd hemoglobin (HbAIe) and ESR were detected. Sixty-two patients were divided into two groups according to levels of ESR and CRP respectively. Each index was compared in each groups. Results The TC/HDL-C, GA and CRP in elevated ESR group were significantly higher than those in normal ESR group(P 〈 0.05,P 〈 0.01 ) ;the HbA1 c, GA and ESR levels in elevated CRP group were significantly higher than those in normal CRP group ( P 〈 0.05, P 〈 0.01 ). Conclusion High levels of inflammatory state in patients of RA combined with T2DM may lead to blood glucose and lipids metabolism disorders, then may increase the risk of cardiovascular disease.
