传染性单核细胞增多症患儿的T细胞免疫改变

摘    要:[目的]探讨传染性单核细胞增多症(IM)患者细胞免疫功能的动态变化规律。[方法]通过流式细胞技术,检测76例确诊IM患者的急性期及恢复期血清中总T淋巴细胞(CD3)、T辅助细胞(CD4)和T抑制细胞(CD8)的水平进行分析,并与正常对照组比较。[结果]急性期CD3^+、CD8^+T淋巴细胞显著升高,CD4^+淋巴细胞和CD4/CD8比值明显下降,存在着细胞免疫功能紊乱。在疾病的恢复期,虽CD8淋巴细胞有所下降,CD4淋巴细胞和CD4/CD8比值升高,但与对照组比较差异有统计学意义,机体的细胞免疫功能紊乱贯穿整个病程。[结论]检测外周血T淋巴细胞亚群表达对评估IM患儿的免疫功能状况、疾病的诊断及病情严重度判断都具有重要的临床意义。

Abstract:[Objective]To explore the dynamic changes of cellular immune function in children with infectious mononucleosis(IM).[Methods]By using the flow cytometry,the antigens of CD3,CD4,CD8 on T lymphocytes in peripheral blood of 76 subjects diagnosed with IM were examined respectively during acute stage and convalescent period.The obtained results were finally compared with those of other healthy children.[Results]During acute stage,the percentages of CD3 lymphocyte and CD8 lymphocyte in peripheral blood increased makedly,while the percentage of CD4 and CD4/CD8 ratio decreased obviously,which suggested that T lymphocyte response disturbance existed in the IM children.Besides,during convalescence stage,though the level of CD8 lymphocyte lowered,the level of CD4 lymphocyte and the CD4/CD8 ratio increased,there were still significantly different compared with healthy children,which suggested that T lymphocyte response disturbance persistently existed in the whole course of the disease.[Conclusions]To detect T lymphocyte subsets in the peripheral blood of IM subjects is of great clinical significance in immune function evaluation of IM children,disease diagnosis and determination of disease severity.
