液体衰减翻转恢复序列(FLAIR)在头颅MR检查中的价值

摘    要:目的 探讨液体衰减翻转序列(FLAIR序列)在头颅MR检查中的应用价值。方法 对106例病人进行了轴位FSE T2和FLAIR序列的检查,25例做了轴位SE T1增强序列。并分别从不同病种和不同病变部位进行了分析。结果 106例病人中,86例有阳性发现,共显示117个病灶。60例脑梗塞病人,FSE T2和FLAIR序列对病变的敏感性相同,但在FLAIR序列能区分梗塞是否为陈旧性。3例转移瘤病人,FLAIR和SE T1增强序列均比FSE T2序列的敏感性高。3例脑脓肿病人和3例脑膜炎病人,FLAIR和SE T1增强序列能显示FSE T1序列不能显示的累及脑室和脑膜的炎症,且FLAIR序列比SE T1增强序列显示的范围大。13例原发性脑肿瘤病人和2例动静脉畸形病人,FSE T2、FLAIR和SE T1增强序列对病变的敏感性相同。1例癫痫病人,FSE T2、SE T1增强序列均为阴性,而FLAIR序列能显示右颞叶近脑沟的高信号病变。结论 在头颅MR检查中,FLAIR序列对邻近脑室、脑池和脑沟的病变较FSE T2更为敏感。

Abstract:Purose To investigate the value of fluid - attenuated inversion - recovery ( FIAIR) sequence in craniocerebral MR imaging. Methods T2 - weighted FSE and FLAIR MR imaging were performed in 106 patients and 25 of 106 patients also underwent contrast - enhanced T1- weighted SE MR imaging. MR imaging of three sequence were compared for lesion detection and characterization. Results 86 of 106 patients had abnormal findings and 117 lesions were detected. T2 - weighted FSE and FLAIR had same sensitivity for detection of cerebral infarction in 60 patients but FLAIR could differentiated acute infarction from liquefied lesion. FLAIR and contrast - enhanced T1 - weighted SE had higher sensitivity than T2- weighted FSE for lesion detection in 3 patients with metastatic tumors, 3 patients with brain abscess involving brain ventricles and meninges and 3 patients with meningitis, but three sequences had same sensitivity in 13 patients with primary brain tumor and 2 patients with AVM. In one patient with epilepsy, FLAIR showed a hyperintesity lesion close to sylvian sulcus in right temporal lobe, but this lesion could not be found in T2 - weighted images and contrast - enhanced T1- weighted images. Conclusion FLAIR is more sensitive than T2 - weighted FSE and contrast - enhanced T1 -weighted SE for detecton of lesions close to brain ventricles, cisterns and sulci.
