二维应变超声心动图技术对猪心肌挫伤后左室收缩功能的评价

摘    要:
目的探讨二维应变超声心动图在评价猪闭合性心肌挫伤后左室收缩功能中的应用价值。方法采用小型撞击器撞击9只实验猪,建立闭合性心肌损伤模型,应用二维应变技术测量撞击前及撞伤后各时间点左室短轴切面的径向应变(RS)、圆周应变(CS)、旋转角度(ROT)及左室射血分数(LVEF)。结果撞伤前左室心肌各节段RS、CS、ROT无统计学差异(P〉0.05);撞伤后0.5h、2h、4h、8h、12h左室心肌各节段RS、CS、ROT均较撞伤前下降(P〈0.05);撞伤后8h、12h LVEF较撞伤前降低(P〈0.05)。结论二维应变技术为定量评价闭合性心肌挫伤后左室收缩功能提供了一种快速、操作简便、无角度依赖的的新方法。

Abstract:
Objective To explore the clinical value of two - dimensional strain echocardiography technique(2D- SE)in left ventricle function evaluation in pigs after closed myocardial contusion. Methods Nine pigs were enrolled in this study. Closed myocardial contusion animal model was successfully established by small impactor. And their RS, CS, ROT and LVEF were measured in the left ventricular short - axis view by 2D strain software before and after the myocardial contusion (MC). Results There was no significant difference among RS, CS and ROT in different segments of left ventricle myocardium before the MC ( P 〉 0.05 ). While 0.5 h, 2 h, 4 h, 8 h, 12 h after the struke , RS, CS and ROT were decreased compared with those before struke ( P 〈 0.05), and 8h and 12h after MC, the LVEF were significantly decreased than that before struke ( P 〈 0.05). Conclusion 2D - SE is a rapid, easy and angle - independent method to quantitative assess left ventricular function of closed myocardial contusion.
