恶性肿瘤患者抗凝治疗前后止凝血功能的改变

目的动态观察恶性肿瘤患者抗凝治疗前后止凝血功能的变化,为在恶性肿瘤患者中抗凝治疗的有效性、安全性提供理论和实验依据。方法对56例恶性肿瘤高危血栓患者在用低相对分子质量肝素（LMWH）抗凝治疗前后进行常规止凝血功能的检测,包括PLT、PT、aPTT、TT、Fbg以及血小板活化、凝血、抗凝血和纤溶指标的分子标志物的检测,包括GMP-140、TF、TAT、TFPI、TpP、PaP、uPA、tPA、PAI-1、D-D含量和凝血因子Ⅱ、V、Ⅶ、Ⅷ、Ⅸ、Ⅹ和Ⅺ活性及抗凝血酶AT活性。结果在恶性肿瘤高危血栓组患者应用LMWH抗凝治疗后出现了Fbg、TF、TAT、TpP和D-D含量的降低（P〈0.05或P〈0.01）,TFPI含量和AT活性的上升（P〈0.05）,与抗凝治疗前相比差异有显著性。而PLT略有下降和PT、aPTT、TT略有延长,但与治疗前相比差异不明显（P〉0.05）。结论 LMWH在恶性肿瘤高危血栓组患者的治疗中可以降低凝血因子的活化,减少凝血的激活,增加机体抗凝血的作用,降低血液高凝状态和血栓形成的风险,从而在恶性肿瘤患者中起到预防静脉血栓栓塞（VTE）的作用。

The Change of Haemostatic Function in Patients with Malignant Tumor Before and After the Anticoagulant Therapy
