支气管平滑肌肉瘤1例并文献复习

目的:探讨支气管平滑肌肉瘤的影像学特征,气管镜检查特点和手术治疗方法。方法:对中国医科大学附属盛京医院的1例和1994年以来国内报道的16例支气管平滑肌肉瘤病例和文献进行回顾性总结。结果:胸片常呈非特异改变,肺野肿块影或团块影最常见,部分患者出现肺纹理增强,肺不张等表现;肺部CT检查常可发现肿块影,气管或支气管管腔狭窄,管壁增厚,纵膈或肺内其他部位淋巴结肿大等改变;气管镜检查均可发现新生物;手术治疗主要以肺叶切除为主。结论:该病影像学改变有一定特征,气管镜活检和术后病理是确诊的主要方法,手术治疗是目前治疗支气管平滑肌肉瘤的主要治疗方法。

Bronchial leiomyosarcoma: Report of one case and review of literature

Objective:To study the iconographic and bronchoscopic features and surgical methods of the bronchial leiomyosarcoma.Methods:A retrospective analysis of 17 cases was made,one was diagnosed in Shengjing Hospital of China Medical University and the domestic clinical data of the other sixteen cases reported since 1994 were studied by literature review.Results:Chest X-ray findings are often non-specific.The mass shadow or lump shadow in lung field can be found commonly,and increased bronchovascular shadow and atelectasis can be found directly in some cases.From chest CT scans we often find mass shadow,luminal stenosis,mucosa thickening,enlargement of lymph nodes in mediastinum or other parts of the lungs and so on.By bronchoscopic examinations,the neoplasm can be found.The chief surgical treatment is lobectomy.Conclusion:There are some iconographic and bronchoscopic features in bronchial leiomyosarcoma.The bronchoscopic biopsy and pathology of postsurgery are the main methods to diagnose this disease.Currently,surgery is mainly the treatment of bronchial leiomyosarcoma.
