腹腔镜辅助改良原位肛门成形术在超低位直肠癌保肛术中的临床应用

目的观察腹腔镜辅助改良原位肛门成形术的可行性、安全性及临床疗效。方法对行手术治疗的超低位直肠癌的病例资料进行回顾性分析,其中对照组22例运用腹腔镜辅助改良PARKS手术保肛,实验组20例采用腹腔镜辅助改良原位肛门成形术,比较术中、术后及愈后指标。结果两组在手术时间、术中出血量、住院天数、住院花费、早期肠内营养实施天数、避免吻合相关并发症的发生情况等方面差异有统计学意义（P〈0.05）,在清扫淋巴结数量、术后肛门功能和总生存率上差异无统计学意义（P〉0.05）。结论腹腔镜辅助改良原位肛门成形术安全可行,能够缩短手术时间,减少术后并发症发生,加速术后恢复,缩短住院时间。

Laparoscopic assisted modified in situ anal plasty in anus preserving operation for low rectal cancer in clinical application

Objective To observe the feasibility,safety and clinical efficacy of a modified technique in laparoscopic-assisted in-situ anoplasty.Methods The clinical data of patients with ultra-low rectal cancer performed surgery were retrospectively analyzed.Twenty-two patients were treated with laparoscopic-assisted modified PARKS in the control group,20 patients were treated with a modified technique in laparoscopic-assisted in-situ anoplasty in the experimental group.Preoperative and postoperative prognostic indicators were compared.Results There were significant differences in operative time,blood loss,length of stay,hospital spending,the implementation of early enteral nutrition and avoiding anastomosis-related complications(P < 0.05),while no significant differences in the number of harvested lymph nodes,postoperative anal sphincter function and overall survival rate were found(P > 0.05).Conclusions This modified technique in laparoscopic-assisted in-situ anoplasty is safe and feasible,which can decrease the operation time and postoperative complications,improve postoperative recovery and decrease hospital stays.
