肺炎型肺癌的CT分析

目的通过分析肺炎型肺癌的CT特征，提高本病的诊断水平。方法对15例病理证实的肺炎型肺癌的CT资料进行回顾性分析。结果除1例两肺弥漫分布外，其余14例CT均表现为肺叶实变，叶间裂向外膨隆。7例部分病灶整体或周边伴有磨玻璃样改变。14例可见支气管充气征，充气的支气管多无明显扩张，呈枯枝状，较僵直，粗细不均，并有挤压推移。8例增强后有4例可见“血管造影”征，显示的血管僵直、管壁毛糙、粗细不均匀。5例CT随访，病灶有不同程度的增大，密度变致密。结论肺炎型肺癌的CT表现有一定的特征性，结合临床有助于正确的诊断。

CT study of pneumonic-type lung carcinoma

Objective To improve the diagnostic level of pneumonic-type lung carcinoma by studying CT characteristics. Methods CT data of 15 patients with pathologically proved pneumonic-type lung carcinoma were analyzed retrospectively. Results Except in 1 patient with bilateral diffuse lesions, the CT appearances in remaining 14 patients were lobar consolidation with bulging of the interlobar fissure. Ground-glass opacities were found in the whole or the circumference of the lesions in 7 patients. 14 patients had leafless tree like, irregular, stretching, squeezing of the air-filled bronchus within the area of consolidation, most of them were not dilation. CT angiogram sign was identified in 4 of 8 patients after intravenous contrast administration, showing the irregular, stretching vessels. Follow-up was made in 5 patients, and the lesions became somewhat larger in size and higher in density. Conclusion CT appearances of pneumonic-type lung carcinoma have some characteristics. Combining clinical information, CT is useful for the correct diagnosis.
