腹腔镜下子宫全切术与腹式子宫全切术的比较

目的通过与腹式子宫全切术比较，探讨腹腔镜下子宫全切术的临床疗效。方法选择2008年1月～2011年4月在我院进行子宫全切术的患者42例，随机分为研究组和对照组各21例。研究组采用腹腔镜下子宫全切术，对照组采用腹式子宫全切术，比较两组术中和术后恢复情况。结果与对照组比较，研究组平均术中出血量少，平均肛门排气时间早，平均住院天数少，差异有高度统计学意义（P〈0．01）。研究组手术前后白细胞计数和中性粒细胞比较差异无统计学意义（P〉0．05），对照组差异有统计学意义（P〈0．01或P〈0．05）。术后两组自细胞计数和中性粒细胞百分比比较，差异有统计学意义（P〈0．01或P〈0．05）。结论腹腔镜手术用于子宫全切术具有创伤小、术后恢复快等优点，且对患者的免疫功能影响较小。

Comparsion of laparoscopy with laparotomy for uterectomy

Objective To discuss the clinical effect of laparoscopy for uterectomy compared with laparotomy.Methods Forty-two cases for uterectomy from Jan 2008 to April 2011 were divided into study group and control group.Study group was treated with laparoscopy,and control group was treated with laparotomy.The intraoperation and postoperation condition were observed and compared between two groups.Results Compared with control group,the mean peri-operation bleeding of study group was less,the mean anal exhaust time was earlier,the mean hospital stay was shorter.There was significant difference between two groups(P < 0.01).The leukocyte and neutrophile granulocyte of study group had no significant difference between preoperation and postoperation(P > 0.05),while in the control group had significant difference(P < 0.01 or P < 0.05).The leukocyte and neutrophile granulocyte had significant difference between two group after operation(P < 0.01 or P < 0.05).Conclusion Laparoscopy for uterectomy has advantage of minimal invasion,resume fast,and affect little on immunologic function.
