抗人球蛋白试验的微柱凝集法与传统试管法在自身免疫性溶血性贫血检测中的对比研究

摘    要:
目的探讨微柱凝集法(Microtubecolumnagglutinationtechnology,MCAT)抗人球蛋白试验(antiglobulintest,AGT)在自身免疫性溶血性贫血(autoimmunehemolyticanemia,AIHA)诊断中的临床意义。方法收集146例临床疑为AIHA患者的外周血标本,采用微柱凝集法和传统试管法(conventionaltubetechnique,CTT)进行抗人球蛋白试验,并对2种方法AGT试验检测AIHA结果进行比较分析。结果46例I临床最终诊断AIHA病例中,MCAT法检出DAT阳性44例(95.7%),IAT阳性12例(26.1%);CTT法检出DAT阳性22例(47.8%),IAT阳性2例(4.3%);在MCAT法检出的44例患者中,IgG+C3d阳性23例(52.3%),单纯IgG阳性8例(18.2%),单纯C3d阳性13例(29.5%)。在CTT法检出的22例患者中,IgG+C3d阳性12例(54.5%),单纯IgG阳性3例(13.6%),单纯C3d阳性7例(31.8%)。MCAT法阳性检出率明显高于CTT法,差异有统计学意义(Pd0.05)。结论微柱凝集法抗人球蛋白试验较传统试管法操作更简单,更敏感,更容易标准化,结果判读客观。

Abstract:
Objective To evaluate the clinic value of Microtube column agglutination technology antiglobu- lin test in dignosis of autoimmune hemolytic anemia (AIHA). Methods Specimens of 146 suspected AIHA patients were performed antiglobulin test by Microtube column agglutination technology and con- ventional tube. The results of the two tests have been comparatively analyzed. Results Among 46 patients who were diagnosed as AIHA, under the test of MCAT, DAT positive cases are 44 (95.7%) and IAT positive cases were 12(26. 1%); while under the test of MCAT, DAT positive cases were 22 cases (47.8%) and IAT positive cases were 2 cases (4.3~). Among 44 cases detected by MCAT, IgG+C3d was positive in 23 cases (52.3%) , IgG was positive in 8 cases (18.2%) and C3d was positive in 13 cases (29.5%). The positive rate of MCAT was significantly higher than the CTT method, the difference was statistically significant (P 〈0.05). It has been obviously observed that detection rate in positive of Microtube column agglutination technology Compared with conventional tube, more sensitive, easier to perform and is much higher. the microtube col standardized in d The difference is statistically significant.
