椎旁注射治疗腰椎间盘突出症

目的观察椎旁注射法治疗腰椎间盘突出症的效果。方法在病变的棘突间隙与上一棘突间隙连线的中上1／3处，向患侧旁开2～2，5cm进行穿刺，使穿刺针经横突间韧带达椎旁间隙，注入消炎镇痛液。治疗2～3个月后进行随访，按照标准评定疗效。结果260例中优172例，良55例，可15例，差18例，有效率93．1％，优良率87．3％。结论椎旁注射，体表标志清楚，穿刺成功率高，治疗效果良好，并发症少。

Paravertebral injection for the treatment of protrusion of intervertebral disc

Objective To assess the therapeutic effects of paravertebral injection on the protrusion of lumbar intervertebral disc. Methods 260 patients with protrusion of lumbar intervertebral disc were received para-vertebral injection. The patients were in prone position, located the interspinous space of protrusive disc by the image examination and clinical manifestation, marked it and the upper one. The puncture was through the intersection of upper and middle 1/3 in the line between the two interspinous space, paramedian 2.5cm. The puncture needle was inserted vertically until vertebral lamina touched, then needle withdrew to subcutaneous and tilted outward. As the needle penetrated the intertransversal ligment, there was a sudden loss of resistance. Analgesic mixture 9-15 ml containing lidocain, triamcinolone, vitamin B12and vitamin B6 was injected. The therapeutic effects were scored after 2-3 months. Results Of 260 patients received paravertebral injection, the effect of 172 cases was excellent grade, 55 cases good, 15 cases improvement and 18 cases bad. The effective rate ( except bad) was 67.3 %. Conclusion Paravertebral injection for the treatment of protrusion of lumbar intervertebral disc is a simple and safe mothod, the surface marker of puncture clear, the rate of success higher, complication less and therapeutic effect better.
