氯胺酮联合利多卡因与芬太尼在小儿静脉麻醉中的比较

摘    要:
目的探讨小儿静脉麻醉的最佳药物配伍及用药方法。方法40例拟行非气管插管的静脉麻醉患儿随机分为两组,利多卡因组和芬太尼组,每组20例。两组以同等剂量的氯胺酮和东莨菪碱分别复合利多卡因和芬太尼,给予负荷量后,以同样的速度微量泵输注。记录静脉用药后、术中、术后等时点的收缩压、心率、呼吸频率、脉搏、血氧饱和度以及恢复期苏醒时间。结果两组患儿用药后和手术中收缩压、心率组内组间比较无显著差异(P>0.05);两组患儿用药后和手术中呼吸频率、脉搏、血氧饱和度组间比较有显著差异(P<0.01);两组苏醒时间有显著差异(P<0.01)。结论氯胺酮复合利多卡因微量注射泵输入用于小儿静脉麻醉镇痛完善,呼吸功能稳定,术后苏醒迅速,提高了围手术期的安全性。

Abstract:
Objective To discuss the best compatibility of medicine and the way of using medicine in the shot time intravenous anesthesia for children.Methods Forty children who need not trachea intubation but need intravenous anesthesia were divided into two groups randomly 20 cases each group.In the based on used scopolamine,the ketamine combined lidocaine or fentanyl was used with same dosage by infusion pump at same speed. The blood pressure(BP),pulse rate (HR),frequency of respioration (FR),pulse oxygen saturation(sPO_2) and the analepsia time were observesd.Results The BP and HR after anesthesia and during operationt there were no significant difference between the two groups (P>0.05). HR and sPO2 after anesthesia and during operationt there were significant difference between the two groups (P<0.01).There was significant difference in analeptic time between two groups (P<0.01).Conclusion Application of ketamine combined lidocaine in the intravenous anesthesia for children by infusion pump possess ideal analgetic effect and could kape ideal function of breath,and the analepsia quickly after operation and the operation become more safe.
