综合性护理对脑血栓患者下肢深静脉血栓形成的效果

目的探讨综合性护理对脑血栓患者下肢深静脉血栓形成(DVT)的给予干效果。方法将脑血栓伴下肢DVT的94例患者随机分为观察组和对照组,每组各47例。两组患者均采用常规溶栓抗凝治疗,对照组给予常规护理,观察组在常规护理的基础上同时给予综合性护理,评价两组患者的临床疗效,测定血浆凝血酶原时间(PT)、活化部分凝血活酶时间(a PTT)、纤维蛋白原(Fbg)、D二聚体(D-D);监测患肢平面周径、消肿时间、下床活动时间、以及并发症发生情况。结果观察组患者的治疗效果显著优于对照组,观察组的总有效率(70.21%)显著高于对照组(48.94%)(P0.05),Fbg以及D-D的水平显著低于对照组(P0.05),患肢的平面周径(髌骨下缘15 cm以及髌骨上缘15 cm)显著小于对照组(P0.05),患肢消肿时间和患者的下床活动时间显著短于对照组(P0.05),并发症发生率显著低于对照组(P0.05)。结论综合性护理有助于脑血栓患者下肢DVT的康复。

Effect of Comprehensive Nursing on Lower Extremity deep Venous Thrombosis in Patients with Cerebral Thrombosis

Objective To investigate the effect of comprehensive nursing on lower extremity deep venous thrombosis ( DVT) in patients with cerebral thrombosis .Methods A total of 94 patients with cerebral thrombosis accompanied by lower extremity DVT were randomly divided into observation group and control group ,with 47 cases of each group .Patients in the two groups were treated with conventional thrombolytic and anticoagulant therapy .In the control group ,patients received routine nursing .In contrast ,the observation group received both routine nursing and comprehensive nursing .The clinical efficacy of the two groups was evaluated .The plasma levels of prothrombin time ( PT ) , activated partial thromboplastin time ( aPTT ) , fibrinogen(Fbg)and two D-dimer(D-D)of all patients were tested.The perimeter of the affected limb,the time of swelling ,ambulation and complications after treatment were recorded .Results The clinical efficacy for the observation group was significantly better than that observed in control group ,reflected by the total efficiency in the observation group ( 70.21%) was significantly higher than in the control group ( 48.94%) ( P<0.05 ) , the levels of Fbg and D-D were significantly lower than the showed in the control group ( P<0.05 );The perimeter of the affected limb ( patellar bone edge 15 cm and the upper edge 15 cm) were significantly lower than in the control group ( P<0.05 );the time of limb swelling and ambulation were significantly shorter than in the control group ( P<0.05 );and the complication rate was also significantly lower than control group ( P<0.05).Conclusion Comprehensive nursing was very effective for clinical rehabilitation of lower extremity DVT in patients with cerebral thrombosis .
