妊娠合并卵巢肿瘤68例的诊断与治疗

目的探讨妊娠合并卵巢肿瘤的诊断与治疗方法。方法对2009年1月至2012年1月我院妇产科收治的68例妊娠合并卵巢肿瘤患者采用常规妇科检查、B超检查和肿瘤标志物AFP监测,以手术治疗为主,辅以药物治疗。结果 68例妊娠合并卵巢肿瘤患者中,良性肿瘤67例(98.53%),交界性肿瘤1例(1.47%),未发现恶性卵巢肿瘤;2例卵巢肿瘤蒂扭转行附件切除术,余66例在孕晚期行剖宫产,同时行卵巢肿瘤剥除术或卵巢部分切除术,治疗效果满意。结论孕早期常规妇检和超声检查是妊娠期卵巢肿瘤诊断及监测的重要手段,综合考虑孕周、肿瘤部位、大小、性质等因素,选择适宜的手术治疗时机和手术方式,可以有效地防止并发症,且不影响妊娠结局,对于确保孕妇和胎儿安全具有重要的临床意义。

Diagnosis and Treatment of 68 Cases of Ovarian Tumors in Pregnancy

Objective To explore the diagnosis and treatment of ovarian tumors in pregnancy.Methods 68 cases of pregnant patients with ovarian tumors in our hospital from January 2009 to January 2012 were diagnosed with regular gynecological examination,B-ultrasound examination and tumor marker AFP detection,and were treated with surgery,supplemented by drug therapy.Results In 68 cases of pregnant patients with ovarian tumors,benign tumors were 67 cases(98.53%),borderline tumor was one case(1.47%),without malignant ovarian tumors;2 cases of ovarian torsion underwent adnexectomy,the remaining 66 cases underwent cesarean section as well as ovarian cystectomy or partial ovarian resection,with satisfied treatment effect.Conclusions Regular gynecological and B-ultrasound examinations are important diagnosis and monitoring of early pregnancy with ovarian tumors;the appropriate surgical treatment timing and surgical approach will be chosen by comprehensively considering the factors such as gestational weeks,tumor location,size,and nature,to effectively prevent the complications without affecting the outcome of pregnancy,which has important clinical significance for ensuring the safety of pregnant women and fetus.
