全胸腔镜下肺叶切除术治疗早期非小细胞肺癌的临床观察

目的探讨全胸腔镜下肺叶切除术治疗早期非小细胞肺癌（NSCLC）的临床疗效。方法将92例早期NSCLC患者随机分为对照组和观察组，每组46例。对照组患者采用传统开胸手术治疗，观察组患者采用全胸腔镜下肺叶切除术治疗。比较两组患者的手术情况、术后并发症发生率及术后生存情况。结果观察组患者术中出血量、术后胸腔引流液体量、手术切口长度、下床活动时间、住院时间均优于对照组，差异有统计学意义（P〈0．05）。观察组患者术后并发症发生率少于对照组，差异有统计学意义（P〈0．05）。观察组患者与对照组淋巴结清扫数目、术后1年无瘤生存率差异无统计学意义（P〉0．05）。结论全胸腔镜下肺叶切除术手术切口小，术中出血量少，术后并发症发生率低，手术效果与传统开胸手术相当，应用方便，值得临床推广应用。

Clinical observation of complete video-assisted thoracoscopic lobectomy in the treatment of early non-small cell lung cancer

Objective To investigate the clinical effect of complete video-assisted thoracoscopic lobectomy in the treatment of early non-small cell lung cancer （NSCLC）. Methods Ninety two patients with early NSCLC were randomly divided into control group and observation group. The control group patients underwent traditional open lobectomy, observation group patients underwent Complete video-assisted thoracoscopic lobectomy. Surgery situation, the rate of postoperative complication and the postoperative survival of patients were compared between these two groups. Results The peri-operative bleeding, pleural effusion volume, incision size, the leaving bed time and the hospital stay in the observation group were significantly better than those in the control group （P 〈 0. 05）. The incidence of postoperative complications in the observation group was less than the control group, the difference was statistically significant （ P 〈 0. 05 ）. The number of lymph node and the one year disease free survival after one year between these two growths did not have significant difference （P 〉 0. 05）. Conclusions Complete video-assisted thoracoscopic lobectomy had smaller incision, less peri-operative bleeding, lower incidence of postoperative complications and was easy to use, the clinical effect was similar to traditional open surgery. Therefore, Complete video-assisted thoracoscopic lobectomy is worthy for clinical application.
