乌司他丁联合血必净对急性呼吸窘迫综合征患者炎症反应及氧化应激状态的影响

摘    要:目的探讨乌司他丁联合血必净对急性呼吸窘迫综合征(ARDS)患者炎症反应及氧化应激状态的影响。方法将2009年9月~2012年11月在郴州市第一人民医院南院ICU接受治疗的50例ARDS患者随机分为治疗组(25例)和对照组(25例)。两组ARDS患者均接受常规治疗,治疗组患者在此基础上给予乌司他丁联合血必净;于治疗前及治疗72 h后,检测ARDS患者血清C-反应蛋白(CRP)、白细胞介素-1(IL-1)、白细胞介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)及超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)及丙二醛(MDA)含量。结果治疗72 h后,对照组血清CRP、IL-1及TNF-α的水平明显高于治疗组,差异有统计学意义(P〈0.05),而两组血清IL-10水平差异无统计学意义(P〉0.05);对照组血清SOD和GSH-Px的含量明显低于治疗组,而对照组MDA含量高于治疗组,差异有统计学意义(P〈0.05)。结论乌司他丁联合血必净可减轻ARDS患者炎症反应及氧化应激状态。

Abstract:Objective To investigate the effect of Ulinastatin combined with Xuebijing for inflammatory response and oxidative stress of patients with ARDS.Methods From September 2009 to November 2012,50 patients treated in the ICU of Southern Branch of the First People's Hospital of Chenzhou City whom with acute respiratory distress syndrome(ARDS) were randomly divided into treatment group(25 cases) and control group(25 cases).The two groups were treated by conventional therapy,and the patients of treatment group were given Ulinastatin combined with Xuebijing which excepted the conventional therapy;Before treatment and after 72 h,the serum C-reactive protein(CRP),leuko cyte interleukin-1(IL-1),leukocyte interleukin-10(IL-10),tumor necrosis factor-α(TNF-α) and super oxide dismutase(SOD),glutathione peroxidase(GSH-Px) and malondialdehyde(MDA) of ARDS patients were detected.Results After 72 h of treatment,the serum CRP,IL-1 and TNF-α level of control group were significantly higher than treatment group,with statistically significant differences(P〈 0.05),while serum IL-10 level of the two groups had no significant difference(P〉 0.05);the serum SOD and GSH-Px level of the control group were significantly lower than treatment group,and MDA content was higher than the treatment group,the differences were statistically significant(P〈 0.05).Conclusion Ulinastatin combined with Xuebijing can reduce the inflammatory response and oxidative stress of patients with ARDS.
