温州市首轮医疗服务价格调整效果分析——以某省属三甲医院甲状腺癌病种为例

目的:分析医院在执行医疗服务价格调整方案后,甲状腺癌根治(单侧)手术患者住院费用构成情况及变化原因,并提出建议。方法:选取价格调整前后共计1859例住院患者为研究对象,分析价格调整前后平均住院费用及其构成。结果:价格调整后,病人负担的住院费用并未明显增加,费用结构更趋合理,药占比降低;化验费、大型设备检查费不减反增;体现医护人员技术劳务价值的项目价格与成本仍然不匹配。结论:应确定合理的收入与成本比价关系,并以此分层分类动态调整价格,优化费用结构,腾出空间提高技术劳务项目收费水平。

Analysis on the Effect of First Round Price Adjustment of Medical Service in Wenzhou--Take the Thyroid Cancer Disease of a Provincial Grade Three A hospital as an Example

Objective To analyze the composition of hospitalization expenses and the reasons for changes in patients undergoing radical(unilateral)surgery for thyroid cancer after the implementation of medical service price adjustment plan,and offer recommendations.Methods A total of 1859 inpatients before and after price adjustment were selected as the research object.The average hospitalization costs before and after price adjustment and their composition were analyzed.Results After the price adjustment,the hospitalization costs borne by patients did not increase significantly,the fee structure became more reasonable,and the drug proportion decreased;the laboratory fees and large-scale equipment inspection fees increased;the project prices and costs that reflected the value of medical personnel's technical services still does not match.Conclusion The reasonable income-cost ratio relationship should be determined.And the price should be adjusted dynamically based on this hierarchical classification,optimize the cost structure,and save cost for higher technical labor service charges.
