胎粪吸入综合征与婴幼儿哮喘发病的相关性研究

摘    要:目的探究胎粪吸入综合征(MAS)与婴幼儿哮喘发病的相关性。方法收集2004年1月至2008年5月在我院就诊的160例MAS患儿(MAS组),同期足月正常新生儿105例作为对照组,比较两组出生时血清过敏性IgE抗体,随访后肺炎支原体抗体(MP-IgM)、呼吸道合胞病毒抗体(RSV-IgM)、肺功能变化和哮喘发病差异,了解MAS与婴幼儿哮喘发病的相关性。结果 MAS组出生血清过敏性IgE抗体含量明显高于对照组(P<0.05);两组患儿发病时肺炎支原体抗体和呼吸道合胞病毒抗体无统计学意义(P>0.05);两组患儿第一次发生喘息未治疗前行肺功能检中指标tPTEF/tE、vPTEF/vE比较具有统计学意义(P<0.05);MAS组患儿哮喘发生率高于对照组(χ2=7.79,P<0.05),且Pearson列联系数C为0.3551,表明MAS与哮喘存在关联性。结论胎粪吸入综合征与婴幼儿哮喘发生及程度密切相关,建议有针对性地进行孕期早期干预,防止胎粪污染及胎粪宫内致敏,对降低婴幼儿喘息有一定意义。

Abstract:Objective To discuss the relationship between meconium aspiration syndrome(MAS)and infants asthma.Methods 160 cases of infant MAS received by our hospital between January 2004 and May 2008 were collected,105 cases of normal neonatal in the same period were selected as the control group,allergy IgE antibodies in serum at birth of the two groups were compared;mycoplasma pneumoniae antibodies(MP-IgM),respiratory syncytial virus antibody(RSV-IgM),changes of pulmonary function and different rates of asthma were followed up,the correlation between MAS and the incidence of asthma in infants and young children was studied.Results The serum IgE level of the MAS group was significantly increased comparing with control group(P<0.05).The level of MP-IgM and RSV-IgM had no statistical differences(P>0.05)between two groups;the indexes in lung function check of tPTEF/tE,and vPTEF/vE between two groups before first breathing had statistical significance(P<0.05);the asthma incidence of MAS group was higher than the control group with significant difference(χ2=7.79,P<0.05),and the Pearson contingency coeffecient was 0.3551,which indicated the correlation between MAS and asthma.Conclusion MAS is closely related with the incidence and severity of asthma in infants,it is recommended that targeted early intervention during pregnancy should be done to prevent meconium in utero and meconium pollution sensitization,which has certain significance to the reduction of infant asthma.
