GP方案联合放疗治疗晚期非小细胞肺癌的临床研究

目的探讨放疗联合国产GP(吉西他滨+顺铂)治疗晚期非小细胞肺癌(NSCLC)的近期疗效及安全性。方法将62例符合条件的晚期NSCLC患者根据治疗适应证和家属意见分为观察组和对照组,各31例。观察组采用放疗加GP方案化疗,放疗先大野DT 36～40 Gy,后缩野到肿瘤区加量至DT 64 Gy。化疗采用吉西他滨1200 mg,静脉滴注,第1、8天,顺铂20 mg,静脉滴注,2次/周,共4周;对照组采用单纯放疗。治疗结束后两组进行疗效评价。结果观察组完全缓解率显著高于对照组(P<0.05);两组患者Ⅱ～Ⅳ度白细胞减少、血红蛋白减少、血小板减少、恶心、呕吐发生率比较差异有统计学意义(P<0.05)。结论 GP方案联合放疗治疗晚期NSCLC近期疗效较好,能明显提高患者的生活质量。

Clinical Effect of GP Regimen Chemotherapy Combined with Radiotherapy in Advanced Non-small Cell Lung Cancer

Objective To evaluate the recent effect and safety of GP(gemcitabine + cisplatin) regimen chemotherapy combined with radiotherapy in advanced non-small cell lung cancer(NSCLC).Methods According to the treatment indications and families opinions,a total of 62 patients with NSCLC were divided into observation group and control group,31 cases in each group.Observation group received radiotherapy(firstly magna-field DT 36-40 Gy,then narrow down and increase the intensity to DT 64 Gy) plus GP regimen chemotherapy as follows:gemcitabine(GEM) 1200 mg iv on day 1 and 8,cisplatin 20 mg iv 2 times a week,for 4weeks;control group was only given radio therapy.Therapeutic effect of the two groups were assessed two days after the completion of the treatment.Results The complete remission rate of observation group was significantly higher than that of control group(P<0.05);differences of Ⅱ-Ⅳ neutropenia,hemoglobin decrease,thrombocytopenia,the incidence of malignant vomiting between the two groups were statistically significant(P<0.05).Conclusion GP regimen chemotherapy combined with radiotherapy has good short-term effect treating the advanced NSCLC,and can obviously improve the patients’ quality of life.
