支气管结核的多层螺旋CT诊断分析

摘    要:
目的分析支气管结核的多层螺旋CT表现,探讨其在支气管结核诊断中的作用。方法回顾性分析42例支气管结核患者经细菌学检查、纤维支气管镜活检及手术病理证实的支气管结核的16层螺旋CT表现。结果支气管结核的主要CT表现为支气管结核好发于上叶支气管,病变范围较广泛,可多支受累;支气管管壁多为不规则增厚,管腔不规则狭窄,少数可完全闭塞,可引起肺不张;部分支气管壁可见钙化;多数病例肺内可见支气管播散灶。结论多层螺旋CT可提高支气管结核的诊断率,对支气管结核的临床诊断具有重要价值。

Abstract:
Objective To investigate the multi-slice spiral CT imaging findings of bronchial tuberculosis and to evaluate its value in the diagnosis of the disease.Methods A retrospective analysis of 16-slice spiral CT imaging findings of 42 patients with bronchial tuberculosis confirmed by bacteriology,bronchoscopy,or operation was conducted.Results Main CT features were as follows:multiple bronchi were invaded,frequently in the upper lobes.The bronchial walls were always irregular thickening.The bronchial lumen was often stenosed irregularly,some may be completetly blocked,causing pulmonary atelectasis.The bronchial dissemination focus were frequent.Some calcification of the bronchial wall were observed.Conclusion Multi-slice spiral CT scan,which can increase the diagnosis rate,and is an important and helpful method for the clinical diagnosis of the patients with bronchial tuberculosis.
