N端脑钠肽前体评估维持性血液透析患者容量负荷的临床研究

目的 探讨N端脑钠肽前体（NT-proBNP）与维持性血液透析患者（MHD）容量负荷关系的临床意义.方法 选择我院血液透析中心MHD患者120例,测定血液透析前及透析后2小时血浆NT-proBN水平,根据血液透析超滤量与干体重百分比从低到高将患者分为4组,观察血浆NT-proBNP水平变化与MHD患者容量负荷变化之间的关系.结果 MHD患者血液透析前后血浆NT-proBNP水平较健康者高,血浆NT-proBNP水平变化与容量负荷变化呈正相关.结论 血浆NT-proBNP水平可作为临床上评估MHD患者容量负荷增加的一个有效参考指标.

Evaluation of NT-proBNP to Volume Load in Patients with Maintenance Hemodialysis

Objective To investigate the relationship between NT-proBNP and volume load in maintenance hemodialysis （MHD） patients. Methods 120 cases of patients with MHD in hemodialysis center were selected. Plasma NT-proBNP levels before and after two hours of the hemodialysis were detected. The patients were divided into four groups according to the percentage of hemodialysis ultrafiltration and stem body, the changes of plasma NT-proBNP levels and volume load in MHD patients were observed. Results The plasma NT-proBNP levels before and aider the hemodialysis of MHD patients were significantly higher than those of the healthy people （P 〈0.05）, and the changes of plasma NT-proBNP levels had positive correlation with volume load （P〈0.05）. Conclusions Plasma NT-proBNP can be a effective reference index in evaluating the increase of volume load of MHD patients in clinic.
