剖宫产疤痕妊娠8例临床分析

目的:探讨剖宫产疤痕妊娠的诊断和治疗。方法:对8例剖宫产疤痕妊娠患者的临床资料进行回顾性分析。结果:8例中有3例初诊误诊,误诊率为37.5%。8例中7例均经阴道彩色多普勒超声检查确诊,另1例术后确诊。7例患者药物治疗加B超引导下清宫术治疗,均治愈;1例诊断为宫内早孕11周行钳刮术,术中、术后大出血达1 700 ml而行急诊开腹子宫全切除术。结论:剖宫产疤痕妊娠误诊率高,超声检查是主要的诊断方法,药物治疗加B超引导下清宫术可有效杀胚,减少大出血风险且避免子宫切除,是治疗剖宫产疤痕妊娠的有效方法。

Clinical analysis of cesarean scar pregnancy with 8 Cases

Objective To study the diagnosis and treatment of cesarean scar pregnancy.Method The clinicaI data of 8 patients with cesearean scar pregnancy were analyzed retrospectiveIy.Results 3 out of 8 patients were premilinary misdiagnosed,and the misdiagnostic rate was 37.5%.Seven patients were diagnosed by transvaginal color doppler ultrasound,and the other was diagnosed after operation.Seven patients were treated by medication and suction dilatation and curettage with B-ultrasound guiding.The other patient was hemorrhaging in dilatation and curettage and finaIly had hysterectomy.Conclusion The misdiagnostic rate of cesarean scar pregnancy is high.ultrasound is the main diagnostic choice.The medication and suction dilatation and curettage with B-ultrasound guiding is an effective method.
