胃泌素在胃炎与胃癌中表达的差异及意义

摘    要:目的探讨胃炎与胃癌中,胃体、胃窦组织内胃泌素表达的水平的差异与意义。方法收集我院2009~2010年胃癌手术患者21例,选取同期行胃镜检查的慢性浅表性胃炎患者20例作为对照,免疫组化法检测组织内胃泌素的表达,IPP软件测定胃泌素累积光密度。结果不同性别及不同年龄胃泌素累积光密度的对数差异无显著性(P值分别为0.151和0.482)。胃炎组胃泌素累积光密度对数的中位数在胃体为2.58(1.60~4.24),胃窦为4.55(3.27~5.23)(Z=4.166,P=0.000);癌组织与癌旁组织比较,胃体处癌为1.46(0.00~4.61),癌旁为3.51(0.00~5.30)(Z=3.089,P=0.002),胃窦处癌为0.00(0.00~4.23),癌旁为4.89(4.26~5.55)(Z=4.916,P=0.000);癌旁组织与胃炎组相应部位比较,胃体处癌旁为3.51(0.00~5.30),胃炎组为2.58(1.60~4.24)(Z=2.397,P=0.017),胃窦处癌旁为4.89(4.26~5.55),胃炎组为4.55(3.27~5.23)(Z=2.193,P=0.028);癌与胃炎组比较,胃体处癌为1.46(0.00~4.61),胃炎组为2.58(1.60~4.24)(Z=2.578,P=0.010),胃窦处癌为0.00(0.00~4.23),胃炎组为4.55(3.27~5.23)(Z=4.828,P=0.000)。结论不同性别及不同年龄胃泌素表达差异无显著性。胃泌素在胃窦处表达高于胃体。无论胃体胃窦,癌旁、胃炎、癌组织中胃泌素表达呈递减趋势。胃泌素表达水平的变化可能为胃癌发生前的一种状态。
