摘    要:目的 探讨个体化改良Abumi椎弓根螺钉置入技术在下颈椎临床应用的效果.方法 对9例须行下颈椎后路椎弓根固定术的患者,术前行薄层三维CT检查,并对图像进行分析、测量,确定手术椎体椎弓根进钉的投影点及角度;采用改良的Abumi椎弓根螺钉置入技术置入颈椎椎弓根螺钉;术后再行三维CT扫描,观察每个椎体椎弓根螺钉置入的准确性.结果 9例患者下颈椎共置入48枚椎弓根螺钉;术后CT复查优37枚,良6枚,差5枚,成功率89.6%,无并发症发生.结论 利用个体化改良Abumi椎弓根螺钉置入技术在下颈椎徒手置入颈椎椎弓根螺钉安全、可靠,适合于在临床推广应用.

Improved Individual Abumi Pedicle Screw Placement in Lower Cervical Spine

Abstract:Objective To explore the clinical application of the improved individual Abumi pediele screw placement in the lower cervical spine.Methods Thin-section three-dimensional CT was performed before posterior pedicle screw fixation of lower cervical spine in 9 patients. Images were analyzed and measuredto confirm the enter points and angles of the screws. The pedide screws were implanted with improved Abumi pedicle screw placement technique. The accuracy of pedicle screw placement was evaluated by three-dimensional CT after surgery. Results A total of 48 pedicle screws were implanted in the'9 patients. Postoperative CT scans showed that the result was excellent in 37 pediele screws, good in 6, ahd poor in 5. The success rate was 89.6%. No complications occurred in all patients. Conclusion The individual free-hand pedicle screw placement is safe and reliable in the lower cervical spine and can be generalized in clinical oractice.
