应用躯体移动评估表指导髋部骨折病人的体位护理

目的 通过躯体移动评估指导髋部骨折病人实施体位护理,解除因躯体移动障碍及卧床不适给病人带来的痛苦。方法 采用一种简易快捷的躯体移动评估表,对病人住院第l天、手术后第l天及出院前l天进行评估,根据评估分值将病人划分为安全(3分)、较安全(2分)、易发(1分)和高发(0分)4种人群．进行有针对性的体位护理。结果 215例病人通过实施体位护理,住院期间出现皮肤压红17例．平均1．5d内消退．出现第二期褥疮5例,平均5d内愈合。治疗期间患肢保持了正确牵引,体位符合治疗要求。通过正确的躯体移动方法,缓解了卧床后引起的腰背部不适。结论 评估表充分体现了病人的主要特点;应用评估表评估体现了体位护理的专科特色,缓解了因卧床给病人带来的不适;进行评估的过程增进了护患沟通;通过规范管理确保体位护理行之有效;护士在体位护理过程中得到了锻炼和提高。

Body movement evaluation directs body position nursing of hip fracture patients

Objective To relieve the painsof hip fracture patientsfrom body movement barriersand lie-in-bed upsetsusing body movement evaluation to direct body position nursing.MethodsAn easy form wasapplied to evaluate body movementsof the patientson every1st day at admission,after operation and before discharge.Based on the scores,the patientswere classified into four cate-gories:quite safe(3scores),safe(2scores),susceptibility(1score),frequent onset(0score)and the body position nursing wasperti-nently performed.ResultsAmong the215patientsreceiving body position nursing,17manifested with skin-pressed red,which fad-ed in average1.5days,5with degree2bedsore,which washealed in average5days.During treatment,the diseased limbskept right retracted,the position fitting to the treatment.The proper body movement relieved the discomfort at the back.Conclusion The eval-uation form embodiesthe main featuresof patients.Itsapplication showsthe feature of body position nursing asspecial nursing,which alleviatesthe discomfort in patientslying for a long time.The processof evaluation promotesthe communication between nurs-esand patients.The standard management ensuresthe effect of body position nursing and nursesobtain exercisesand development in the course of performing body position nursing.
