腹腔镜与开腹直肠癌前切术前瞻性临床对照研究

目的:探讨腹腔镜直肠癌前切术的可行性及近期疗效。方法:回顾分析2004年2月至2008年4月我院行腹腔镜直肠癌前切术18例及开腹直肠癌前切术25例患者的临床资料,比较两组患者手术及术后情况。结果:腹腔镜组与开腹组手术时间分别为(248.06±77.48)min、(180.6±16.91)min,差异有统计学意义(P=0.00);术中出血量分别为(44.44±46.17)ml、(107.60±79.23)ml,差异有统计学意义(P=0.03);淋巴结清除数分别为(11.5±2.75)枚、(11.16±3.02)枚,差异无统计学意义(P=0.604);术后胃肠道功能恢复时间分别为(2.0±0.77)d、(4.28±1.21)d,差异有统计学意义(P=0.033)。结论:腹腔镜直肠癌根治术安全有效,患者出血少,创伤小,术后康复快,根治效果达到开腹手术的水平。

Prospectively clinical control study of laparoscopic and open anterior resection of rectal cancer

Objective:To investigate the feasibility and short-term therapeutic effect of laparoscopic and open anterior resection of rectal cancer.Methods:The clinical data of 18 cases underwent laparoscopic anterior resection of rectal cancer and 25 cases underwent open anterior resection of rectal cancer in our hospital from Feb.2004 to Apr.2008 were analyzed retrospectively.Results:The mean operation time of laparoscopic group and open group was(248.06±77.48)min and(180.6±16.91)min respectively,the difference between two groups was significant(P=0.00);The intra-operative blood loss of laparoscopic group was obviously less than that of open group(44.44±46.17)ml vs.(107.60±79.23)ml,(P=0.03).There was no significant difference between the number of cleared lymph node(11.5±2.75)vs.(11.16 ±3.02),(P=0.604).The gastrointestinal function of laparoscopic group recovered more quickly than open group(2.0±0.7)d vs.(4.28±1.21)d,(P=0.033).Conclusions:Laparoscopic radical resection of rectal cancer is safe and effective.It has advantages of less blood loss and trauma,earlier postoperative recovery,and similar radical effect as laparotomy.
