急腹症误诊为急性阑尾炎的临床分析

目的:探讨临床上急腹症误诊为急性阑尾炎的原因。方法:回顾性分析2008年1月～2011年1月期间22例急腹症误诊为急性阑尾炎患者的临床资料。结果:22例患者均以右下腹痛为主诉,查体右下腹均有压痛。误诊后经手术、病理等方法确诊,其中胃十二指肠穿孔7例,卵巢囊肿4例,化脓性胆管炎3例,胆结石3例,卵巢蒂扭转2例,异位妊娠2例,急性坏死性胰腺炎1例。全部误诊患者中行手术治疗16例,非手术治疗6例。全部患者经手术及药物治疗后,均痊愈出院。结论:对急性阑尾炎患者需根据年龄、性别认真询问病史,并进行全面正规的体格检查,进行必要的B超、X线片、腹穿、尿妊娠试验等辅助检查,避免误诊。

Clinical Analysis of Acute Abdomen Misdiagnosed as Acute Appendicitis

Objective:To explore the reasons for clinical acute abdomen misdiagnosed as acute appendicitis.Methods:A retrospective analysis of the period January 2008 to January 2011,22 cases of acute abdomen misdiagnosed as acute appendicitis in patients with clinical data.Results:22 patients are in the right lower abdominal pain chief complaint of right lower quadrant of the examination are tenderness.Misdiagnosed diagnosed after the surgery,pathology,and gastroduodenitis perforation in seven cases,four cases of ovarian cysts,three cases of suppurative cholangitis,three cases of gallstones,two cases of ovarian torsion,two cases of ectopic pregnancy,acute one case of necrotizing pancreatitis.All misdiagnosed patients Bank of surgical treatment of 16 cases,non-surgical treatment of six cases.All patients after surgery and drug therapy were cured.Conclusion:Given the regular physical examination and comprehensive the necessary B-ultrasound,X-ray,paracentesis,urine pregnancy test and other examinations for Patients who with acute appendicitis based on age,gender,serious history to avoid misdiagnosis.
